A TMT or treadmill stress test records your ECG while you walk on a treadmill, showing how the heart behaves under exertion. It is one of the first tests used to check for blocked arteries in patients with chest pain or breathlessness.

TMT helps decide whether chest symptoms are cardiac and whether further tests such as angiography are needed.
Come with a light meal at least two hours earlier, wear comfortable shoes and loose clothing, and bring previous ECG, echo and medicine lists. Some medicines such as beta-blockers may need to be paused — confirm with the clinic before your appointment.
Electrodes are placed on the chest and your ECG, heart rate and blood pressure are monitored while treadmill speed and slope increase every three minutes. The test stops when the target heart rate is reached or if symptoms appear. A positive or inconclusive TMT is usually followed by an echo or coronary angiography.

It is not painful. It is performed under continuous monitoring by a cardiologist, and the test is stopped immediately if you develop chest pain, giddiness or ECG changes.

It suggests reduced blood supply to the heart muscle during exertion. It does not confirm a blockage on its own — an echo and, if needed, coronary angiography are advised next.
